Please also supply a company letterhead, along with photocopies of one proof of company identification and one proof of personal identification. (Proof of company identification includes Certificate of Incorporation, a bank statement in your company’s name etc. Proof of personal identification includes a photo card driving license, recent utility bill etc.).

I/we hereby apply for a credit account with Mellow Marsh Software Ltd., and agree to abide by the terms and conditions of sale and business. I/we also agree to comply with the payment terms which are strictly 30 days from date of invoice. Our preferred payment method is via BACS, however cheque, credit/debit card and cash are also available subject to our discretion. By applying for a credit account, you agree that we will perform a credit reference search upon you, your company, and your company's director(s), partner(s) and/or owner(s). You also agree that we will monitor your credit profile periodically, and that your credit limit is subject to change or removal without prior notification. You also agree that we can remove your right to a credit account and require immediate settlement of account without prior notification.
